By submitting this form, I accept full responsibility for my / my child / children’s participation in YMCA Gymnastics activities. I understand that we must follow all safety instructions, rules, and guidelines provided by YMCA staff. I acknowledge that the YMCA will not be held liable for any injury, loss, or damage sustained during these activities. I hereby release, indemnify, and hold harmless the YMCA, its officers, directors, employees, agents, and volunteers from any and all claims, demands, or actions arising out of or in connection with my /child/ children’s participation in the gymnastics activities at the YMCA. This release applies to all claims arising from negligence, injury, or other causes. I also accept YMCA's Terms & Conditions for Casual and Trial Classes.
The details collected on this form are being collected to allow the Y to provide you with access to the activity in which you are applying for your child to participate. Failure to provide this information will mean that we will not be able to host your child for this activity. Your information will be stored on Vision6 servers located in Australia and on internal Y Queensland databases and systems.
