By submitting this form, I accept full responsibility for my / my child / children’s participation in YMCA gymnastics activities. I understand that we must follow all safety instructions, rules, and guidelines provided by YMCA staff. I acknowledge that the YMCA will not be held liable for any injury, loss, or damage sustained during these activities. I hereby release, indemnify, and hold harmless the YMCA, its officers, directors, employees, agents, and volunteers from any and all claims, demands, or actions arising out of or in connection with my /child/ children’s participation in the gymnastics activities at the YMCA. This release applies to all claims arising from negligence, injury, or other causes. I also accept YMCA's Terms & Conditions for Casual and Trial Classes.
